This innovative solution, aligned with value-based care models, identifies Social Determinants of Health (SDoH) to assess patients' non-clinical needs and enhance the prediction of care costs associated with clinical conditions. It ensures patients receive timely and appropriate care while optimizing capacity and cost-effectiveness for healthcare providers and organizations.


Predictive risk modeling plays a crucial role in identifying patients in need of substantial medical assistance, considering that a small portion of the population accounts for a significant portion of healthcare spending. Persistent's PHM Solution utilizes Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights to pinpoint Social Determinants of Health (SDoH) using Electronic Health Records (EHR) data. Unlike other PHM solutions reliant solely on third-party or survey data, Persistent's solution accurately extracts up to 80% of vital patient information from unstructured clinical notes within EHRs. This innovative approach aims to enhance patient outcomes by offering more timely and intelligent care recommendations, boasting higher accuracy, improved scalability, and enhanced compliance compared to existing natural language processing-based methods.

Moreover, Persistent's PHM Solution aids physicians or caregivers in suggesting personalized interventions via community-based programs and care management teams, aiming to prevent emergency department visits, avoidable readmissions, and prolonged hospital stays. This solution has demonstrated a reduction in 30-day readmissions by up to 60% and maintains ongoing monitoring of correlations and outcomes to ensure high-quality patient engagement.
